A New Culture of Innovative Collaboration for Diabetes Service Redesign

In a challenging financial environment, NHS Trusts in the Kirklees area redesigned their diabetes service in order to deliver high quality levels of care closer to patients’ homes and promoting patient self-management and self-care.

Led by Dr Warren Gillibrand, the service design was developed following a diabetes care review, stakeholder consultation and training needs analysis. The delivery of the study was aided by the University of Huddersfield and a number of pharmaceutical companies.

Starting in 2009, there is currently full involvement from specialist teams and 74 practitioners who
have attended University programmes.

Six practices are accredited with the diabetes LES piloting of the Self-Care Handbook and patient service needs were identified using care planning data.

New working patterns have developed between the NHS and the private sector, and the introduction of e-consultation has facilitated the installation of a clinical record system into secondary care allowing the sharing of patient information. More information is available online.
